• 774_00【Urinary bladder; Bladder膀胱 Vesica urinaria】 Organ located beneath the peritoneum in the lesser pelvis posterior to the pubic symphysis. Its size varies depending on fullness, with the urge to evacuate the bladder occurring at about 350 ml. Even at maximum distension it remains below the level of the navel.
  　→(膀胱は腎臓で産生され尿管によって送られる尿を約３５０～５００mlまたはそれ以上を一時的に貯える。平滑筋よりなり弾性に富む尿の貯留器官。膀胱は骨盤腔のもっとも前部にあり、恥骨の後ろに位置する。軽度に充満する時には、四面体を呈し、頂にあたるところを膀胱尖といい、錐体の底部にあたるところを膀胱底と呼ぶ。尖と底との間を膀胱体と呼ぶ。)

• 774_01【Prostate; Prostate gland; Prostatic glands前立腺 Prostata】 Chestnut-sized tubuloalveolar gland below the urinary bladder and having a smooth surface. It surrounds the urethra.
  　→(前立腺は膀胱底の下に密接し、骨盤底の上にのる30～50個の胞状管状腺である。男性の尿道起始部を取り囲んで栗の実に似た形をしている。強靱な結合組織の被膜に被われる。外周に近い部部分は色がやや暗く見え（いわゆる外腺exogland）、中心部は白っぽい（いわゆる内腺endogland）。Prostataはpro（前に）stata（立つもの）という意味である。古代ギリシャでは永来ヒトの前に立って護衛する人をprostatesといったそうである。前立腺を初めて記載し、prostatesと名付けたのは、アレキサンドリア学派のエラシストラトスErasistratus(BC310-250)である。前立腺が膀胱の前に立ってこれを守っていると考えたのだろう。日本では、以前には摂護腺と呼ばれていたが、ギリシャ語の直訳の前立腺が現在では正規の用語になっている。)

• 774_02【Prostatic urethra of male urethra前立腺部（尿道の）；尿道前立腺部 Pars prostatica】 Part of the male urethra that passes through the prostate.
  　→(前立腺の内部を貫く部で、長さ約3cmである。この部の内腔は尿道のうちで最も広く拡張しやすい。前立腺部の後壁には膀胱垂からつづく縦走する粘膜の高まりがある。この高まりを尿道稜という。尿道稜につづく後壁の中央部は精丘という紡錘状の隆起となり、精丘の頂に盲管状小陥凹あがある。この陥凹を前立腺小室という。)

• 774_04【Ejaculatory duct射精管 Ductus ejaculatorius】 Last segment of the ductus deferens, located within the prostate, narrowing similarly to a nozzle.
  　→(射精管は前立腺内にある、狭いノズル様精管最終部。射精は性行為時などにおける性的興奮が増すにしたがい陰茎亀頭の外尿道口の部分がぬれてくるが、これは尿道球腺の分泌活動が高まったことによる。さらに陰茎亀頭への摩擦刺激、あるいはその他の性的刺激が加重されると今度は交感神経系の活動が起こり、両側の精巣上体管、精管、精嚢、さらには前立腺における交感神経支配下の滑液筋線維群の収縮をまねいて精巣が精嚢および前立腺分泌物とともに尿道前前立腺部へと排出される。次に球海綿体筋のリズミックな収縮が起き、このため尿道は狭められ排出液が球尿道腺やや海綿体部尿道腺からの分泌物と混ざった上体で外尿道口より射出される。そのとき膀胱括約筋は収縮し、精子が膀胱内に流れ込むのを防ぐ。体外に射出された、精子と性器付属腺分泌物との混合液を精液semenと称する。射精は中枢神経内での大きな神経活動を伴う。脊髄まで下行した深頚興奮は胸腰髄節（T1～L2）の交感神経節前ニューロンを刺激する。性器へと向かう交感神経節後ニューロンの軸索突起の多くは第１～２腰部交感神経節内の細胞体より発するものであるが、これよりも下位の交感神経節より発するものもある。これら軸索突起は下下腹神経叢を経由して精管、精嚢、前立腺などへ向かうのである。)

• 774_07【Median umbilical ligament正中臍索；臍尿管索 Ligamentum umbilicale medianum】 Remnant of the obliterated allantois in the median umbilical fold.
  　→(尿膜allantosisは胎生初期に内胚葉性の卵黄嚢上皮から臍帯内に突出する憩室で、とくにトリでは発達して嚢状となり、孵化以前に尿を貯留する。しかし、哺乳動物（ヒト）では、発達がわるく管状で尿膜管urachus, allantonic ductといわれ、臍から膀胱に走るが、退化して線維索、すなわち正中臍索となる。)

• 774_08【Muscular layer of urinary bladder; Muscular coat of urinary bladder筋層（膀胱の） Tunica muscularis vesicae】 The muscular coat of the urinary bladder mainly consists of interwoven bundles of smooth-muscle cells that conform to the degree of its distension. In the trigone region muscle fibers from the urinary bladder overlap with those of the ureter.
  　→(膀胱の筋層は膀胱壁の主部をつくる平滑筋層である。平滑筋は原則的に３層（内縦・中輪・外縦）に配列するが、全体的に網状を呈して３層の区別は必ずしも明瞭ではない。平滑筋は収縮によって膀胱内圧を圧し排尿に与えるので、全体として排尿筋とよばれる。膀胱頚では、筋層の中輪走筋が輪状にとり囲み、膀胱括約筋をつくるといわれたが、このような輪走筋は明らかでなく、膀胱三角の内縦層の平滑筋が膀胱頚から尿道に達している。このような縦走筋が収縮すると、尿道の始部は短くなり、かつその内腔は漏斗状にひろがって、内尿道口が開き尿の排出に与る。膀胱の平滑筋が弛緩すると、縦走筋も弛緩する。同時に膀胱頚にある輪状の弾性線維によって内尿道口は閉ざされるといわれる。)

  　→(膀胱の粘膜は薄く筋層と疎に結合し、膀胱が空虚で収縮するときには、粘膜に不規則なヒダがみられるが、充満拡張時には、粘膜は伸展しヒダは消失して平滑になる。しかし、膀胱底では、粘膜は筋層と強く結合し、表面は収縮・伸展にかかわらず常に平滑である。この部は三角形状を呈し、膀胱三角(図7-15)といわれる。膀胱三角の底辺は後上方にあり、長さ約4cmで、その左右両端に、尿管が開口する(尿管口)。三角の頂点は前下方にあり、ここから尿道が出る(内尿道口)。左右の尿管口の間には、粘膜ヒダ(尿管間ヒダ)が走る。（解剖学講義）)

• 774_17【Uvula of bladder膀胱垂 Uvula vesicae】 Longitudinally oriented prominence on the tip of the trigone of bladder, produced by a venous plexus beneath the mucous membrane in the posterior wall of the urethral orifice.
  　→(　内尿道口は恥骨結合のほぼ中央の高さで、その約5cm後方にある。男性では、内尿道口のすぐ後ろに粘膜の高まりがある。この高まりを膀胱垂uvula vesicaeという。膀胱垂は、膀胱の下にある前立腺の中葉が突隆するために生ずる。　★膀胱垂は、とくに中年以後に前立腺肥大が起こると、著明になる。（解剖学講義）)
